In a significant step forward for dementia care in the United States, PocketRN, a leader in virtual nursing, and Caring Senior Service, a prominent home care company, have announced a national strategic partnership.
This collaboration aims to test a new Medicare dementia care model, the Guiding an Improved Dementia Experience (GUIDE) Model, developed by the Centers for Medicare & Medicaid Services (CMS).
The GUIDE Model: A New Approach to Dementia Care
The GUIDE Model, launched on July 1, 2024, is a groundbreaking initiative designed to provide better support for individuals living with dementia and their caregivers.
It focuses on a holistic, person-centered approach to care, with the primary goal of enabling people with dementia to remain in their own homes for as long as possible.
Under this new model, participants will have access to a wide range of services, including:
- Comprehensive Care: Personalized assessments and care plans tailored to individual needs.
- 24/7 Support: Round-the-clock access to an interdisciplinary care team or a dedicated helpline.
- Care Coordination: A dedicated Care Navigator to help access both clinical and non-clinical services, such as meals and transportation.
- Caregiver Support: Respite services to provide temporary relief for caregivers, along with education and training on best practices for dementia care.
PocketRN and Caring Senior Service: A Powerful Partnership
This partnership brings together two industry leaders with a shared commitment to improving the lives of people with dementia and their families.
PocketRN will be one of nearly 400 participants in the GUIDE Model, building Dementia Care Programs (DCPs) across the country.
Jenna Morgenstern-Gaines, CEO of PocketRN, expressed her enthusiasm for the partnership, stating, “We couldn’t be more thrilled to bring our revolutionary Nurse for Life model to the millions of dementia patients and families who need it most.”
PocketRN’s unique model provides patients and families with unwavering support from a dedicated, trusted nurse at no cost to them.
Caring Senior Service, with its extensive experience in providing in-home care for individuals with dementia, is a natural fit for this partnership.
Jeff Salter, founder and CEO of Caring Senior Service, emphasized the importance of the GUIDE Model’s holistic approach, saying, “The GUIDE Model focuses on a holistic approach to dementia care with the goal of keeping individuals in their own homes for as long as possible.”
What This Means for People with Dementia and Their Caregivers
